Medical Social Workers (MSW)

Relevar’s MSW’s will lessen anxiety in patients and their families when faced with overwhelming questions arising during illness and the rehabilitation process. Our MSW’s assess physical, social and emotional issues related to the patients unique care requirements. Then MSW’s collaborate with the Relevar Recovery Team to develop and implement care in the home. Relevar’s MSW’s act as a liaison to other community resources to provide an efficient, comfortable integration of care. Relevar’s MSW’s assistance includes:

• Provides Counseling
• Assists with Social or Financial Difficulties Arising from Medical Condition
• Provides Access to Community Resources
